Rule 12b-10 -- Requirements as to Proper Form
Every statement or report shall be on the form prescribed therefor by the Commission,
as in effect on the date of filing. Any statement or report shall be deemed to be
filed on the proper form unless objection to the form is made by the Commission within
thirty days after the date of filing.
